How to Apply for NPI Application/update form?

What is NPI?

The National Provider Identifier (NPI) is a numerical identifier assigned to physicians and other health care providers by the Center for Medicare and Medicaid Services (CMS). It is a permanent 10 digit number assigned to you for your entire medical career.

In order to bill the Faculty for the services ordered or referred by you, it is necessary for you to apply for and obtain NPI. This number will be required after you enter the practice, as it is used to bill for your services.

NPI application/update form

The National Provider Identifier (NPI) Application/Update Form (CMS-10114) has been modified to provide additional guidance on how to complete the form correctly. The revised form was recently approved by the Office of Management and Budget (OMB).

The revised version of the npi application form is currently made available for viewing and downloading on the CMS form website /Medicare/CMS-Forms/CMS-Forms/downloads/CMS10114.pdf (PDF). The link of the downloadable form can also be found under ‘Download’ below.

Health care providers can apply for NPI in one of three ways:

  1. Apply through National Planning and Provider Enumeration System (NPPES).
  2. Complete, sign, and mail a paper NPI Application/Update Form to-

  3. Allow an Electronic File Interchange Organization (EFIO) to send application data through a bulk enumeration process.

How to Apply for NPI Application online?

– Then you can log in to your account if you have a “user-id” and “password” available. If you are a new user you need to “create your new account”.

– After that, Provide the required information in the registration form.

  • State / Province / Territory
  • Zip Code
  • Primary E-mail Address

– Once you complete the application, please submit the application.
